    ABSTRACT:

    Background: The use of High voltage stimulation in patients with pressure ulcers is very important, and thus the scientific evidence for its effectiveness needs to be evaluated through a systematic review. Objective: To provide updated evidence-based guidance for High voltage stimulation effects on the healing of pressure ulcers. Data sources: Pub Med (Medline), Ovid, Physiotherapy Evidence Database (Pedro), Google scholar website, Cochrane Central Register of Controlled Trials, Electron library of Cairo University, Science direct website were searched from their earliest records up to December. Data extraction: Template was created to systematically code the demographic, methodological, and miscellaneous variables of each RCT. The Physiotherapy Evidence Database (Pedro) scale was used to evaluate the study quality. Data synthesis: Five studies included 200 participants in total. Best evidence synthesis was applied to summarize the outcomes, which were wound Surface Area (WSA), Wound tracing measures, Planimetry of Congruent. Conclusions: The current level of evidence to support the effectiveness of electric stimulation (High voltage stimulation) on pressure ulcers healing is strong. As according to this review there is support enough to use electric stimulation (High voltage stimulation) in treatment of pressure ulcers to reduce wound surface area (WSA) in different stages of pressure ulcers and accelerate their healing and decrease the mortality rate of a patient with pressure ulcers.

    ABSTRACT:

    The study of monomodal language in discourse linguistics needs to be combined with other fields, such as visual, auditory and media studies. Diet is closely related to culture, and diet culture is an important part of the splendid culture of China, which is the concentrated expression of people's production and lifestyle for thousands of years. It reflects people’s attitude towards life, and also represents the spirit and temperament of a nation. Chinese culture is of long standing and well established, and cultures in different fields have their own particularities. The documentary a bite of China, as a multimodal discourse that includes text, pictures, music, photography and other art forms, is dedicated to introducing the rich and colorful food culture of China. The results lend support to relevance theory. In addition, this paper probes into the food culture of a bite of China from the perspectives of vision, taste and touch. Lastly, the result makes a brief analysis of the Chinese food culture attempt to explore how the film reflects Chinese food culture from different perspectives via certain modes. The study provides a new perspective for discourse analysis composed of multiple symbols and offers readers a framework for theoretical analysis of culture. It helps individuals to improve their ability to read multimodal discourses, and enables people to study symbolic resources in a more in-depth way.

    Background: Diabetic nephropathy is one of the major microvascular complications of diabetes which eventually manifests into end-stage renal disease. One-third of the diabetic population is prone to develop nephropathy and it represents the major cause of morbidity and mortality. Objective: The purpose of this study was to find out the effect aerobic training on urinary albumin excretion protein in type 2 diabetes mellitus. Methods: the study was conducted on twenty patients, their age ranged from 45 to 55 years. They participated inmoderate aerobic exercise (50-70%of MHR) for 30-40 minutes on electronic bicycle ergometer, 3 session per week for 12 weeks in addition to their medical treatment. Micro albumin urea, HBA1C, Fasting blood glucose, blood pressure was measured before and after the study. Results: statistical analysis showed a significant improvement (decrease) in HbA1C By 1.26%, Fasting blood glucose by 5.21%, Systolic & diastolic blood pressure by 4.78% &6.79% and albuminuria level by 9.68%. Conclusion: it is recommended for patients of diabetic nephropathy to practice aerobic exercise to avoid diabetic complication

    Presentation of the case. The case of a 25 year old woman diagnosed with osteosarcoma of the left proximal tibia is described, which was treated with en bloc resection with a non-conventional prosthesis operated at 8 years old at the Children's Hospital of Mexico; It first required replacement prosthesis at the age of 12 years, the second at 15 years old. At 17 years of age presents new material failure remaining 2 years without support. The patient goes to the consultation of bone tumors at the Naval Medical Center at the age of 19 years to develop a massive metallosis with skin pigmentation and localized edema in his left knee, so new tumor prosthesis is placed. Subsequently patient evolved satisfactorily without data infection, rejection or tumor reactivity prosthesis; The patient is now independent of their daily activities. It indicates that the failure of the prosthesis should be tried revision surgery as soon as possible, discard any infectious process or reaction osteosynthesis material, to avoid injury to soft tissue.

    The management of Le Fort-type fractures, their surgical repair and the management of the airway are a challenge for the surgeon and the anesthesiologist. Although orotracheal intubation is the most frequently used route in securing airway, it interferes with the surgical field, as an alternative nasotracheal intubation is commonly used. However, nasotracheal intubation is contraindicated in cases of skull-base trauma, therefore the tracheostomy is a good route for securing the airway, nevertheless, this procedure is associated with various complications. In 1986 submental intubation was described for the first time, since then it has been used as an attractive option in the control of the airway management for this type of situations. We present the case of a patient with multiple fractures of the maxillofacial complex, in whom tracheostomy was avoided and the submental intubation technique was chosen.

    Background: Chronic low back pain (CLBP) is a debilitating condition that persists despite the lack of tissue damage and an effective management is still lacking. CLBP is a multifactorial disorder comprising psychosocial factors like pain catastrophization, fear avoidance and central sensitization. Objectives: to investigate the effect of Cognitive Behavioural Therapy on CLBP patients with Central sensitization. Methods: This randomized clinical study was conducted on thirty patients of both genders having chronic low back pain with central sensitization, their age ranged from 20-37years old. The 30 patients were divided into two groups. The first group received Cognitive Behavioural Therapy comprising of one session neurophysiology education, one biofeedback relaxation session and three sessions comprising functional training exercises. The second group received conventional physiotherapy treatment comprising 12 sessions of TENS and core strengthening exercises over the course of 4 weeks. Results: The results of this study showed a significant improvement in pain intensity according to the NPRS of 30.02% in the CBT group while only 5.82% in the conventional physiotherapy group. Conclusion: Cognitive Behavioural Therapy has a significant effect on chronic low back pain, disability and fear avoidance behavior related to its central sensitivity aspects which are neglected by conventional treatment physiotherapy. Larger studies are required to establish the best feasible treatment protocol.

    Purpose of the study: To investigate the effect of mirror therapy on Complex Regional Pain Syndrome (CRPS) post wrist burn. Methods: Thirty patients were randomly divided into two equal groups; Group A (study group) and Group B (control group). Group (A) received conventional hand progressive exercise program (active, and active resisted hand exercises)in addition, the same exercises were done by the sound hand in front of mirror for 15 min in the first 2 weeks and 30 min in the last 2 weeks. Group (B) received only conventional hand progressive exercise program. The total duration of treatment was four weeks (30 min, 3 sessions per week). Methods of assessment included visual analogue scale (VAS) to assess pain and hand held dynamometer to assess hand grip and pinch, pretreatment and post treatment (after four weeks). Results: The results showed a significant improvement in VAS and hand grip as the percentage of improvement of VAS in groups A and B was 58.65% and 33.9% respectively, while in hand grip strength was 39.55% and 16.36% respectively. However, there was a significant improvement in VAS and hand grip in group A compared to group B (P=0.001), also we reported a significance increase in pinch strength as the percentage of improvement in groups A and B was 75.16% and 33.56% respectively with more significant improvement in group A compared to group B (P=0.0001). Conclusion: From the finding of the current study we concluded that mirror therapy exercise program is an effective, safe, relatively inexpensive, simple and available modality in decreasing pain and improving function level.

    Background: Splenectomy is a surgical intervention commonly performed at hospitals as a cause of splenic trauma. Aerobic exercise increases blood flow to contracting muscles, causes hypercoagulability and vasoconstriction in the splenic area, hence the portal vein diameter and blood flow velocity decrease. Aim: to study the effect of aerobic exercise on portal venous system in splenectomized patients. Material and Methods: Thirty adult male participated in this investigation were divided into two groups, control group received traditional treatment (encourage ROM, cough techniques and ambulation), study group received aerobic exercise (run for twenty minutes on treadmill at a velocity of 6km/h and ten –degree elevation for four weeks). We compared the white blood cells count, portal vein diameter, portal venous blood flow velocity and functional activities before and after exercise. Results: After exercise, the white blood cells count was significantly increased and portal vein diameter with portal venous blood flow were significantly reduced in study group(p<0.001), but no statistically differences in two groups before and after exercise in functional activities. Conclusion: Aerobic exercise in splenectomized individuals cause decrease splenic blood flow which increase blood viscosity and decease portal vein diameter.

    Teaching competence and classroom management strategies enhance a teacher’s capability to establish and promote an environment that is impartial, accepting, and understanding of diverse students, ideas, circumstances, and experiences. The quest for associating these factors to students' performance has been verified to be crucial in maintaining high standards and implementing best strategies in all classrooms. The main purpose of this research was to assess the association of the teachers’ teaching competence and classroom management strategies to students’ academic performance of Zamboanga del Sur National High School, Pagadian City. This descriptive-correlational type of research utilized the survey questionnaires and school documents as the key to generating the necessary data. The results of this investigation showed that the overall teaching competence of the teachers was very high. Similarly, classroom management strategies of the teachers were also very high. Using the Pearson correlation test, this study asserted that there is a significant relationship between teaching competence and students’ academic achievement. Moreover, this analysis claimed that a significant relationship is established between teachers’ classroom management strategies and students’ performance. Thus, this research provides strong evidence that the decisive role of the teachers interms of manifesting teaching competence and employing classroom management strategies is very crucial in the promotion of the educational successof the students. Thus, teachers should continue to upgrade themselves for the reason that teaching, learning, and the whole teaching-learning process is very dynamic.

    ABSTRACT:

    This study explores the concept of children’s perception toward activities. The study took place at a nursery setting in Hull in the UK. The paper begins by explaining the meaning of children’s voices, and the many ways to tune to these voices which can lead to production of desirable outcomes for children in general. It then goes on to clarify the various ways to communicate with children that allows for their participation and free expression of children’s opinions, feelings etc. After that, the Early Years’ Foundation Stage (EYFS) which is the framework for early years practice in the United Kingdom is explored to find out what extent the idea of tuning to children’s voices is supported, and ways it could be applied in their learning and in particular, ways by which children could be included in the evaluation of activities provided for them. Next, the study presents the project carried out in a nursery setting in Hull city which involved developing a method of gathering children’s perceptions about some of the activities which are provided for them during the day. Finally, I draw on some activities to allow children share their feelings, opinions, and responses to ascertain their preferences for activities. It is suggested that this is within children’s rights as stipulated by the EYFS and proper consideration of children’s voices by practitioners will provide them with the opportunity to participate in the evaluation of activities which could lead to desirable outcomes.

    Objective: To investigate traditional and alternative health seeking behavior for mental illnesses, among patients before visiting psychiatry department of Ch Rahmat Memorial Trust and Jinnah Hospital Lahore. Methods, Patients and setting: This cross-sectional multi-method study was conducted from May2017 to Sep. 2017 at two tertiary care and teaching hospitals in Lahore, and comprised patients with sample size 80 presenting with different mental illnesses at psychiatry OPD. Patients were interviewed through a Questionnaire ‘The Short Explanatory Model Interview (SEMI) to answer the research questions. Questionnaire was consisted of both Qualitative, Quantitative, closed and open ended questions. Patients were both male and female genders. Starting age was 16-18 yrs. and then onwards. Statistical analysis was performed using SPSS 20. Results: Mean age of the patients was 27.91 yrs with SD + 10 yrs While 47were male and 33 were females and 95% were muslims Psychiatrist was the first choice in 45% of the patients. While rest 55% followed the non-psychiatric physicians, traditional and religious faith healers. Important reasons to seek help from different non-psychiatric sources included easy accessibility, beliefs in the system and particular healers and good reputation. Mean duration of treatment from traditional healers variedfrom2-3 months while from psychiatric physician was16.6 months. The mean expenditure per visit to get a service from psychiatrist was higher as compared to alternative traditional healers. Conclusion; About half of the patients with mental health problems seek help from psychiatrist. While other equal half go to the alternative traditional system practitioners as faith and spiritual healers. Patients and relatives justify their visits to these traditional healers by multiple reasons. So it is important to sensitize the general and non-psychiatric physicians with early identification and referral of these cases to Psychiatrists
